Below are 2 options for registering for your mission to Peru. Option 1 is for filling out by hand and Option 2 is for filling out on your computer and then printing to provide to your team leader. These forms include:
- Team Member Registration
- Short-Term Mission Team Member Liability Release
- Individual Health Information
- Health Release Information -These must kept on file with your team leader.
- Consent for Medical Treatment
- Parental Consent Form (for Minor Team Members)

- Field Member Expectation Sheet
